Attachment filtering or the content filter could also be sending messages to the quarantine. The logs will tell you for sure what is causing the messages to be routed to the quarantine.

Yes, there should be no issues upgrading from 21.5.3 to 23.5.0.  We reccomend backing up your entire MDaemon directory structure before upgrading, just in case.
